Welcome to Associate Pediatric Clinic


Established in 1922 the Associate Clinic was one of the first medical clinics in Calgary. Over the years the clinic has evolved to include family doctors, and various subspecialties, including our pediatric clinic.

Our Office Policies

In order to continue to meet the needs of our busy pediatric practice and to continue to take good care of our patients and families, the following policies are in effect:

  • Patients who are over 15 minutes late may not be seen that day. You will be invoiced for a late fee, which must be paid before another appointment can be made.
  • Patients who do not show up for their scheduled appointment
    will be charged a fee. The fee must be paid in order to rebook.
    If you don’t attend your appointment, 2 people miss out:
    you & someone else who could have used that time slot.
  • Please cancel your appointments more than 24 hours in
    advance, if possible. Late cancellations (ie: less than 24hours) will be charged a fee. (See below for our late fee price guidelines)
  • As we are a referral-based practice, patients need to be seen at
    least yearly to continue to be part of our clinic. Past that yearly mark, a new referral is required. Please see the referrals page for information on our referrals process.
  • Need a prescription refill? Please ensure you have a scheduled appointment with your physician and have your pharmacy fax us a refill request to bridge medications to your next appointment.

Developmental Screening Form

If your child is 5 years old or less, please complete a developmental screening questionnaire for your first visit to our office. It is usually repeated every 4-6 months. Please talk to your pediatrician if you have any questions.

Child Mental Health Rating Scales

If your child has ADHD, anxiety, depression or other mental health concerns, your pediatrician may ask you to complete online rating scales to assist in diagnosis or monitor their progress. This new website has multiple rating scales available. When filling out the questionnaires, please look for ASSOCIATE PEDIATRICS and not our individual doctor names. Ask your pediatrician which scale is right for your child.
